Preheat your oven to 375°F (190°C). Lightly grease a 9x13 inch baking dish.
In a large bowl, combine the chicken pieces, diced ham, shredded Swiss cheese, condensed cream of chicken soup, milk, and sour cream. Stir until everything is well mixed.
Pour the mixture into the prepared baking dish and spread it out evenly.
In a small bowl, mix the melted butter with the breadcrumbs. Sprinkle this mixture evenly over the top of the casserole.
Bake for 30-35 minutes, or until the casserole is bubbly and the topping is golden brown.
Let the casserole stand for a few minutes before serving.
